The Habif Health and Wellness Center is dedicated to maintaining a healthy campus. We have medical and mental health providers, nursing staff, laboratory services and a pharmacy available on campus in order to provide complete care for your student.
All students entering Washington University in St. Louis must complete a health history form. This requirement is necessary in order for The Habif Health and Wellness Center to provide complete care for your student anytime they may need services and to maintain a healthy campus. Failure to complete this form may prevent your student from registration at the University. This form is available on-line by using the link below.
